What is the process? |

When people can't do what they used to do for themselves as they get older, after an injury or coping with a disability many feel they may need assistance to remain in the home they know and love. If you feel that way, help is available. All you have to do is know how to get it. |

The good news is, it’s a simple two-step process: |
1. Contact us directly, get in touch with your GP or Social Services. |
The first step is getting the relevant authority to understand you have a need. So talk to your GP, Hospital or local Social Services. They will arrange a referral to a professional body – a recognised, regulated and authorised service provider such as Absolute Care Services. If it’s Private Care you are looking for, you know you’re dealing with the right people – and we’ll even tell you if you’re entitled to free services, just ask the team for advice. |

2. Arrange the support you need. |
After you’ve got a referral, we will make an appointment that is convenient to you to assess your needs. During this appointment one of our experienced Area Supervisors will discuss your needs and devise a support plan that is appropriate to you and agree a detailed plan of care. Following the assessment, if we feel that we can help by what we can offer, we’ll provide a Community Support Worker that’s carefully matched to your requirements. |

The Area Supervisor will continue to make contact with you on a regular basis to ensure your needs are met as well as checking to see if changes are needed. They will work very closely with you to adapt your individual service. |
